2024-2025 Transportation Permission Form – each child who will be picked up by The Ministry Machine or other vehicle must have a form completed by the parent.

If you do not have a form completed, you may download the form and complete it. Then text the church stating you’ll have the form(s), one per child, ready when we pick up your child(ren).
