Transportation Permission Form – each child who will be picked up by The Ministry Machine or other vehicle must have a form completed by the parent.

If you already have a ride permission form turned in to the church (each school year requires a new form), you may send a text message to the church phone (409-945-5915) requesting a ride. For Wednesday night, please notify us by Tuesday at 6:00 pm. For Sunday morning, please notify us by 6:00 pm Saturday. This allows us to schedule vehicles and drivers.

If you contact us after these times, we will pick up only if there is space available.
